Ok, I'm going to explain how i made this one 
Open up Photoshop, new image size 250x250px.
Select the rectangle tool, with radius of 4px
Now, use this tool to create something the same size as below:
Thats the basic shape of it, now to add the effects 
Select the layer you've just created, then go to Blending Options (shown below):
Now, select "Inner Shadow" and copy from below:
(You can vary yours to mine, to make it look a little more "yours")
Now, select "Outer Glow" and copy from below:
Now, select "Inner Glow" and copy from below:
Now, select "Bevel & Emboss" and copy from below:
You're almost done, just more step (Optional), now select "Stroke" and copy from below:
Thats it!! You're now done, good luck
