The form that you see is html. The back end that handles it all is php.
Make sure to set form permissions to read & exicute for all. The only one that should be
read/write/exicute is "owner".
It is a simple form but it works and it gives your visitor the oppertunity to have a say about your site.
Change Company Name in HTML file to what you prefer along with images. You can also change the questions in the html file but remem to change them in php file as well.
Add your email in the php file where it says "your email"
If you find a way to make it a little better please share here with the family.
